Painful is a harsh word spoken by an otherwise righteous person. Painful is a warrior growing weary and faltering in battle. Painful is a poor person forced to give charity. And painful are the words of the cruel.

Three of the four are painful precisely because they contradict what we expect of the person doing them: kindness turning harsh, courage failing under pressure, poverty still trying to be generous. The cruel word, though, is simply cruelty acting true to type, so the verse quietly separates disappointment from plain harm.
